As a Hospice RN Case Manager, you will play a vital role in delivering individualized, coordinated hospice care that prioritizes the safety and comfort of patients and their families. Your holistic approach will encompass their physical, emotional, and spiritual well-being, all while enjoying the independence and flexibility of visiting patients in their home environments.
Key DetailsHours: Full-time | Days | General working hours: Monday - Friday, 8:00 AM - 5:00 PM | Participate in an On-Call rotation.
Territory: You will serve a designated area within our service region, primarily focusing on the Southern section of the Twin Cities Metro area.
Compensation & Benefits- Annual Salary Range: $89,419 - $106,620

- Active RN license in the state of MN, in good standing.
- Minimum of 12 months of nursing experience within the last 5 years, or current clinical knowledge if not recently employed.
- If a new graduate, 1200 verified hours of nursing experience within the last 12 months or completion of a company-approved Preceptor Program.
- One year of hospice care experience preferred.
- Reliable transportation, valid driver’s license, and proof of auto insurance required.
- CPR Certification.
