Requirements:

  • Current and valid LPN license in Ohio
  • Minimum of 1 year of experience in a long-term care or similar healthcare setting preferred.
  • Basic Life Support (BLS) certification required.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work effectively in a team-oriented environment and handle multiple priorities.
  • Compassionate and patient-centered approach to care.

Responsibilities:

  • Deliver direct nursing care, including vital sign monitoring and medication administration.
  • Assist residents with activities of daily living (ADLs).
  • Collaborate with the healthcare team to develop and follow care plans.
  • Accurately document patient care and health status changes.
  • Ensure compliance with infection control and facility policies.
  • Maintain a safe, clean, and supportive environment.
  • Provide emotional support and communicate with residents and families.
