Responsibilities:

• Provide direct patient care, including administering medications and monitoring vital signs.
• Assist with daily living activities and ensure patient comfort and safety.
• Collaborate with healthcare teams to develop and implement personalized care plans.
• Educate patients and families on health management and wellness strategies.
• Document patient progress and report any changes in condition to the supervising nurse.
• Maintain compliance with healthcare regulations and company policies.
• Participate in ongoing training and professional development opportunities.
• Build strong relationships with patients and their families to foster trust and support.

Requirements:

• Current LVN license in the state of California.
• Minimum of 1 year of nursing experience, preferably in home health or hospice care.
• Strong clinical assessment skills and attention to detail.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate demeanor with a commitment to patient-centered care.
• Basic Life Support (BLS) certification required.
• Reliable transportation and valid drivers license.
