We are seeking a skilled and empathetic Social Worker (SW) to join our team. In this vital role, you'll help patients and families cope with the emotional, social, and practical challenges that come with a terminal diagnosis�bringing comfort and clarity during their most vulnerable moments./p pstrongWhat You'll Do/strong/p ul liConduct psychosocial assessments of patients and caregivers/li liProvide counseling to patients and families regarding emotional stress, coping, grief, and advance care planning/li liAssist with advance directives, funeral arrangements, and navigating legal/financial decisions/li liIdentify and coordinate with community resources, agencies, and support organizations/li liEducate staff and families about cultural and psychosocial needs/li liDocument patient conditions, interventions, and psychosocial support provided/li liServe as a liaison between families and care teams to ensure coordinated, holistic care/li liParticipate in interdisciplinary team meetings to develop individualized care plans/li liProvide crisis intervention and support for safety concerns (neglect, abuse, self-harm)/li liParticipate in quality improvement initiatives and continuing education/li liParticipate in on-call support as needed/li /ul pstrongQualifications/strong/p ul liBachelor�s degree in Social Work (BSW) required/li liMaster�s degree (MSW) or LCSW strongly preferred/li liAt least 1 year of experience in social work, preferably in hospice, home health, or a community setting/li liKnowledge of end-of-life care, grief support, and cultural sensitivity/li liStrong written and verbal communication skills/li liSelf-motivated, organized, and able to work independently/li liCurrent Basic Life Support (CPR) certification/li liValid driver�s license, auto insurance, and reliable vehicle required/li liPlease note: Legacy Hospice is a drug-free workplace.
