pspan style="color: #000000;"As a Care Coordinator I Bilingual, you will provide case management, mental health and substance use services, crisis intervention, and linkage to community resources for child clients with a history of persistent mental illness or substance use. You will ensure clients maximize the benefits of all interventions in an integrated health care setting and maintain a safe, nurturing environment that fosters hope, dignity, and recovery./span/p

lispan style="color: #000000;"Assists clients and families to locate, access and utilize a full range of health care services, community support groups and resources; establish and maintain housing; access mental and medical care, rehabilitation programs and drug/alcohol services; increase social contacts and engage in constructive activities./spanbr /span style="color: #000000;"Assesses clients’ needs and functioning, presents cases in treatment team meetings and monitors progress as needed./span/li
lispan style="color: #000000;"Acts as client advocate interfacing with housing and support agencies including property owners, support groups, Social Security, Medicare/MediCal, etc./span/li
lispan style="color: #000000;"Develops and maintains rapport and cooperative relationships with community agencies for the benefit of the clients./span/li
lispan style="color: #000000;"Provides individual/group supportive counseling and psycho-education to assist clients in coping with mental illness and/or substance use, promote recovery and develop self-sufficiency./span/li
lispan style="color: #000000;"Provides crisis intervention and facilitates conflict resolution among the program clients as needed; assists clients in problem-solving life situations./span/li
lispan style="color: #000000;"Interfaces with clinical and medical staff in coordinating client care./span/li
lispan style="color: #000000;"Meets Agency client service expectations./span/li
lispan style="color: #000000;"Monitors and maintains documentation of client services; ensures compliance with Agency, county, state and federal policies and procedures, standards and guidelines./span/li
lispan style="color: #000000;"Develops and maintains clinical skills through seminars, workshops, or other forms of continuing education/training related to individuals with a history of mental illness and/or substance use disorders./span/li
lispan style="color: #000000;"Attends and participates in Agency and program/team meetings, individual/group supervision, and related community Agency meetings as required/requested./span/li
lispan style="color: #000000;"Provides social rehabilitation services to clients and their families individually and/or group setting./span/li
lispan style="color: #000000;"Conducts outreach services to generate client referrals./span/li
lispan style="color: #000000;"Participates in community resource and/or health fairs./span/li
lispan style="color: #000000;"Provides parenting services to individuals, groups or the community at large./span/li
lispan style="color: #000000;"If bilingual, assists with interpretation services for medical staff./span/li

lispan style="color: #000000;"Fluency in required language for assigned program (Spanish)./span/li
lispan style="color: #000000;"A minimum of two (2) years of related experience or a bachelor’s degree is required. /span/li
lispan style="color: #000000;"Previous experience working in a social service/mental health environment, preferably with mental health and/or substance use clients./span/li
lispan style="color: #000000;"Current California driver’s license, car insurance, and a driving record acceptable to the Agency’s insurance carrier./span/li
lispan style="color: #000000;"Current knowledge of all job specific skills including crisis intervention and multicultural and socio-economic issues, and documentation standards./span/li
lispan style="color: #000000;"Knowledge of Agency policies and procedures, HIPAA, policies and documentation guidelines, and county, state and federal regulations relating to service programs for individuals and families with a history of mental illness and/or substance use./span/li
lispan style="color: #000000;"Knowledgeable about and actively supports integrated mental health, physical health and substance use treatment for improving care for the whole person./span/li
lispan style="color: #000000;"Basic advocacy skills to include a holistic approach to integrated health care and case management./span/li
lispan style="color: #000000;"Skills to present ideas, information, and viewpoints clearly, both verbally and in writing./span/li
lispan style="color: #000000;"Able to efficiently use the personal computer to include Microsoft Office Suite and Electronic Health Record./span/li
lispan style="color: #000000;"Ability to travel in the community and/or between agency sites to provide services in a variety of settings, including outpatient and field based locations (home, school, etc.)./span/li
lispan style="color: #000000;"Ability to utilize data, outcomes, experience and judgment to make effective business and therapeutic decisions./span/li
