pDescription/ppstrongnbsp;Job Title:/strong Home Health Speech-Language Pathologist; PRN/ppstrongSupervised by:/strong Patient Care Services Director/ppbr//ppstrongJob/strong strongSummary:/strongnbsp;/ppEvaluates, plans and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, language, cognitive-communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions./ppbr//ppstrongDemonstrates Competency in the Following Areas:/strong/pullinbsp;nbsp;Performs accurate and comprehensive speech, language and swallowing assessments. Obtains physician orders for certification and recertification of home speech therapy services./liliEnsures that changes in patients' clinical status are communicated to other healthcare team members and HHA office staff verbally and in writing in a timely manner./liliEnsures that physician orders are communicated to other healthcare team members and HHA staff in a timely, accurate and thorough manner./liliCoordinates care planning and delivery with the physician and other healthcare team members and HHA staff to facilitate optimal patient outcomes./liliDelivers speech therapy modalities appropriate to the patient's clinical status, needs and age. Able to appropriately assess and reassess a patient's pain./liliUtilizes appropriate pain management modalities that include, but are not limited to, physical therapy modalities. Educates the patient and/or family in effective pain management modalities./liliProvides speech therapy modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors./liliSupports and maintains a culture of safety and quality./liliAddresses and supports cultural practices providing such practices do not harm the patient or others, and do not interfere with the planned course of speech therapy./liliParticipates in the HHA's information management and performance improvement programs. Complies with the HHA's policies./liliAccurately completes OASIS data set./liliAdheres to all applicable laws, regulations and standards./liliCompletes and submits necessary documentation in accordance with HHA policy and applicable regulations. Submits verification of completed visits in accordance with HHA policy./liliParticipates in interdisciplinary team conferences in accordance with HHA policy./liliMaintains current knowledge of community resources. Identifies and accesses appropriate external and internal resources to meet patient and/or family needs and to facilitate optimal patient outcomes while home care services are being provided, and for transfer and discharge processes./liliDemonstrates effective time management and organizational skills; provides patient visits as scheduled; completes patient care within the designated time./liliArticulates HHA grievance/concerns procedure./liliArticulates HHA adverse event/incident reporting procedure./liliAssures continued competency by maintaining and updating professional performance, knowledge and skills. nbsp;/li/ulpstrongnbsp;nbsp;Professional Requirements:/strong nbsp;/pulliAdheres to HHA dress code; appearance is neat and clean./liliAdheres to legal practice standards, including applicable federal, state, and local regulations and standards. Completes annual education requirements./liliMaintains patient confidentiality at all times. Wears identification while providing patient care./liliAttends annual review and in-services, as scheduled./liliAttends 12 staff meetings annually or reads and returns all staff meeting minutes./liliMaintains currency of professional knowledge by participating in continuing education offerings./liliParticipates in the organization's performance improvement and continuous quality improvement (CQI) activities. Practices in a manner consistent with the professional and HHA code of ethics./liliRepresents the organization in a positive and professional manner./liliComplies with all organizational policies regarding ethical business practices. Communicates the mission, ethics and goals of the organization./li/ulpbr//ph1br//h1pRequirements/ppstrongRegulatory Requirements:/strong nbsp;nbsp;/pp• Master's or Doctoral Graduate in the area of Speech Therapy or Speech Pathology from an accredited university or college./pp• Certificate of Clinical Competence in Speech-Language Pathology awarded by the American Speech Language and Hearing Association./pp• Current valid state Speech-Language Pathologist emor/em/pp• In the case of an individual who furnishes services in a state which does not license speech-language pathologists:/pp• Has successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating surpervised clinical experience)/pp• Performed not less than nine (9) months of supervised full-time speech-language pathology services after maintaining a Master's or Doctoral degree in speech-language pathology or a related field; emand/em nbsp;/pp• Successfully completed a national examination in speech-language pathology approved by the Secretary.nbsp;/pp• One (1) year of work experience./pp• Prior home health experience preferred. Valid state driver's license and reliable automobile./pp• Proof of current automobile insurance./pp• Current health certificate/physical examination and TB testing./pp• Current BCLS certification./pp• Current liability insurance./ppbr//ppstrongLanguage Skills: nbsp;/strong/pp• Able to communicate effectively in English, both verbally and in writing./pp• Additional languages preferred./ppbr//ppstrongSkills: nbsp;/strong/pp• Understands regulations/standards applicable to home healthcare./pp• Knowledge of general speech therapy modalities and scope of speech therapy practice within the state of Arkansas./pp• Flexible, yet organized, with ability to exercise sound judgment./ppbr//ppstrongPhysical Demands:/strong/ppnbsp;nbsp;/pp• For physical demands of position, including vision, hearing, repetitive motion and environment, see following description./ppbr//ppReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising patient care./ppbr//p
