pThe Re-entry Peer Navigator isstrong /strongresponsible for working with individuals with co-occurring behavioral health, substance misuse disorders and housing needs for those with legal system involvement.nbsp; The Navigator will also work to connect participants to community resources and support to ensure safe and successful community re-entry. The Navigator actively collaborates with partner programs, conducts assessments, identifies needs and barriers, creates re-entry plans, and provides follow up and support./p pstrongResponsibilities amp; Essential Functions:/strong/p pstrongCommunication amp; Connection /strong/p ul liConnect with and assess those returning to the community from incarceration and those engaged with the Criminal Justice System/li liDevelop relationships and provide appropriate referrals for individuals to agencies in the community who can assist in providing resources and overcoming barriers/li liInstruct participants with job seeking and job retention skills/li liCoordinate and participate in workshops and events with participants and partners/li liParticipate in forums and meetings and share information on services nbsp;/li liEnsure the program is in compliance with the Illinois Criminal Justice Information Authority guidelines/li /ul pstrongCase Management /strong/p ul liIdentify appropriate re-entry plans based on evidence and results from needs assessment/li liDevelop, maintain, and update case management plans in a web-based system in a timely manner/li liMeet regularly with participants and update case plans and notes on an ongoing basis/li liAct as a liaison with Goodwill worksite staff and community partners to ensure goals and outcomes are being met./li liCoordinate community resources and supportive services for participants/li liCreate effective solutions for challenges participants face re-entering the workforce/li liCommunicate effectively with participants and program staff/li liProvide follow-up services to assist participants with maintaining community employment/li /ul pstrongIntegrity amp; Trust /strong/p ul liShow acceptance and flexibility in dealing with individuals from disadvantaged social, economic and educational backgrounds/li liMaintain confidentiality and strong ethical practices in working with participants/li liMaintain ethical practices involving protecting company resources of monetary value from loss, waste, misuse, or destruction, and identifying and reporting any such instances/li /ul pstrongAdministrative/strong/p ul liAdminister and utilize the Ohio Risk and Needs Assessment (ORAS), Inventory of Offender Risks Needs and Strength (IORNS), and other risk assessments utilized by different government agencies/li liMaintain participant files and reports in compliance with program requirements/li liHave a Certified Recovery Support Specialist (CRSS) certification or be willing to obtain within six months of hire/li /ul pThe above list of responsibilities is intended to describe the general nature and level of work being performed.
