Social Worker (Hospice & Home Care)

Compensation: Competitive pay based on experience; midpoint of pay scale is approximately $65,000
Schedule: Split between Home Care (20%) and Hospice (80%) teams

Purpose:

Provide social work services, perform psycho-social assessments, and connect individuals and families with appropriate community resources.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments of patients.
  • Develop, implement, and maintain individualized care plans.
  • Deliver strengths-based, family-centered care to promote self-sufficiency.
  • Collaborate with interdisciplinary team members to coordinate patient care.
  • Liaise with external agencies to address patient needs and facilitate resource access.
  • Provide guidance and support based on observations and interviews.
  • Assist in onboarding and mentoring new staff, students, and visitors.
  • Represent the organization in community events and partnerships.
  • Maintain accurate and timely clinical and billing documentation.
  • Monitor patient conditions and report changes to the care team.
  • Ensure a safe and supportive environment for patients.
  • Uphold confidentiality and comply with HIPAA and organizational policies.
  • Participate in quality improvement initiatives and audits.

Qualifications:

  • Bachelor’s degree in social work (BSW) required; master’s degree (MSW) preferred.
  • Minimum of one year of experience in public health or a related setting.
  • Strong knowledge of local community resources and referral processes.
  • Ability to work effectively with diverse populations and adapt to changing schedules and responsibilities.
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office (Word, Excel, Outlook).
  • CPR certification (Healthcare Provider level) required.
  • Mandatory Reporter certification (within 30 days of hire).
  • Valid driver’s license, auto insurance, and reliable transportation required.
  • Successful completion of pre-employment physical, drug screening, TB test, and background check.

Working Conditions:

  • Physical Requirements: Medium work (lifting up to 50 lbs occasionally).
  • Activities: Includes kneeling, standing, walking, pushing, pulling, typing, and visual acuity.
  • Environment: Exposure to chemicals, infectious diseases, and atmospheric conditions.
  • Schedule: Flexibility required to meet program needs.
  • Travel: Required between service locations.
