Saturday 8:00 AM – Monday 8:00 AM | $700 Weekend Stipend + $25–$30/hr DOE
Are you a Licensed Practical Nurse (LPN) looking for meaningful work with flexibility and purpose? SummitWest Care, a mission-driven non-profit home health agency in Grand Junction, is seeking a Weekend On-Call Triage LPN to join our compassionate team.
This vital role allows you to make a real impact while enjoying a compact schedule – work one weekend and help patients get the care they need, when they need it most.
About the Role:
Be on-call from Saturday at 8:00 AM through Monday at 8:00 AM
Respond to patient calls and care concerns over the phone
Provide in-person field visits when necessary
Collaborate with the care team to assess and resolve urgent home health needs
Accurately document triage decisions and patient visits
Compensation & Perks:
$700 flat stipend for the full on-call weekend
$25–$30/hour for any time spent on visits (DOE)
Mileage reimbursement for patient visits
Work from home when not in the field!

What We're Looking For:
Active LPN license in the State of Colorado
Home health experience preferred
Excellent judgment and problem-solving under pressure
Ability to prioritize and communicate effectively
Reliable transportation, valid driver's license, and insurance
Comfortable working independently in a home health setting
