• Full-time schedule, Monday Friday (No weekends!)

• Serve a medically complex, frail population with a holistic, interdisciplinary model

• Lead the delivery of care and support for seniors in a comprehensive, community-based model

• Guide interdisciplinary care teams and mentor providers

• Oversee Quality Assurance/Performance Improvement (QAPI) efforts

• Conduct assessments, treatment planning, and ongoing care management

• Participate in intake evaluations and family conferences

• Ensure compassionate, dignified care for aging adults in various residential settings

• Active FL license

• BC in IM/FM

• DEA & CDS certification

• 1+ year experience working with frail or elderly populations

• Prior leadership or supervisory experience in a clinical setting

• A passion for working in an innovative elder care environment
