RN Case Manager

Job Title: RN – Case Manager
Location: Jackson, MI(49201)

Hourly pay: $55 on W2
Contract Duration: 13 Weeks (High Possibility of extension)
Shifts : Monday–Friday, 8:00 AM – 4:30 PM (Day Shift)

Job Summary:

The RN Case Manager supports the interdisciplinary team in facilitating timely, cost-effective, and quality patient care. Responsibilities include discharge planning, utilization management, care coordination, and supporting documentation accuracy.

Responsibilities

• Facilitation of precertification and payor authorization processes.
• Facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.
• Application of process improvement methodologies in evaluating outcomes of care.
• Coordinating communication with physicians.
• Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient served.
• Ability to demonstrate knowledge of the principles of growth and development as it relates to the different life cycles.

Requirements:

• License: Active Michigan RN License OR LMSW licensure– Required
• Experience:
  • Minimum 2 years of case management experience (Utilization CM not accepted)
  • Discharge planning experience
  • Experience with Care Manager and Epic
