Our employees make all the difference in our success!/b/pp style="text-align:inherit"/pp style="text-align:inherit"/pThe Per Diem - CNA performs personal care, homemaker assistance and defined nursing procedures for patients and their caregivers while under RN direction and supervision.p style="text-align:inherit"/pp style="text-align:inherit"/ppQualifications:/pulliHigh school diploma or GED or an equivalent combination of education and experience/liliCurrent certification as CNA in the state in which employee will be working/liliMinimum of six (6) months of CNA or Home Health Aide experience preferred/liliActive BLS for healthcare professionals from the American Heart Association or Red Cross./liliFor current Hope Employees: BLS should be obtained by January 1, 2025/liliFor current Capital Caring employees: BLS should be obtained by November 1, 2024/liliMobile Driver - Valid driver’s license and automobile insurance per Company policy/liliReliable transportation to meet work schedule/liliAbility to use equipment with visual and auditory mechanisms/liliAbility to communicate (written and verbal) in English/liliAbility to manage the emotional stress of working with and caring for ill patients and their families/liliAbility to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation/liliMust be able to physically access any home or facility in assigned area (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning)/liliMust be able/willing to have contact with individuals with communicable diseases/li/ulp/ppCompetencies:/pulliSatisfactorily complete competency requirements for this position/li/ulp/ppResponsibilities of all employees:/pulliRepresent Company professionally at all times through care delivered and/or services provided to all clients/liliComply with all State, federal and local government regulations, maintaining a strong position against fraud and abuse/liliComply with Company policies, procedures and standard practices/liliObserve Company#39;s health, safety and security practices/liliMaintain the confidentiality of patients, families, colleagues and other sensitive situations within Company/liliUse resources in a fiscally responsible manner/liliPromote Company through participation in community and professional organizations/liliParticipate proactively in improving performance at the organizational, departmental and individual levels • Improve own professional knowledge and skill level/liliAdvance electronic media skills./liliSupport company research and educational activities/liliShare expertise with co-workers both formally and informally/liliParticipate in Quality Assessment and Performance Improvement activities as appropriate for the position./li/ulp/ppJob Responsibilities:/pulliMaintains competencies/contact hours as required by Company and regulations./liliProvides only patient/caregiver services according to written Aide/Caregiver Plan of Care and Company policies and procedure./liliPractices safe and proper body mechanics relative to moving, positioning and transferring patients./liliAssists patient and or caregiver in maintaining a clean and safe home./liliProvides caregiver relief as part of the Plan of Care./liliTransports supplies, lab specimens, documents and other items needed to meet the Plan of Care as requested by the RN./liliUnder the direction or supervision of an RN, the Aide may perform activities that promote activities of daily living./liliContributes to the development of the Plan of Care by reporting observations to RN./liliCommunicates any significant issues or symptoms to the RN or Clinical Manager./liliDuring times of emergencies (i.e. Hurricanes, etc.), the LPN may be required to report to work at a location designated by the company, to ensure continuity of services.
