Certification Details
- Provides direct patient care in accordance with the Nurse Practice Act to assigned patients in the Home Health and/or Hospice setting.
- Collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care.
- Provides physical assessments, IV, infusion, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, and teaching and discharge needs of the patients assigned.
- Utilizes the nursing process based on evidence-based best practices to meet patient care needs.
- Evaluates and adjusts the nursing care process as necessary with consultation as needed and confers with appropriate resource personnel when making ethical/legal/medical dilemma decisions.
- Performs other duties as assigned.
Job Details
- Minimum of two (2) years of acute clinical nursing experience required.
- Clinical home health care experience required for Home Health role.
- Home health or acute clinical nursing experience required for Hospice role.
- Hospice nursing experience preferred.
