Job Summary:

We are seeking a compassionate and dedicated Social Worker to provide psychosocial assessments, care planning, and resource coordination for individuals and families. This role supports clients through a strengths-based, family-centered approach, with a focus on self-sufficiency and community integration.

Key Responsibilities:

• Perform psychosocial evaluations and develop individualized care plans

• Link clients to appropriate community services and support networks

• Collaborate with interdisciplinary teams to coordinate care

• Monitor patient conditions and communicate changes to healthcare providers

• Maintain accurate documentation in compliance with agency and regulatory standards

• Participate in community outreach, staff mentoring, and quality improvement initiatives

Qualifications:

• Bachelor’s degree in Social Work (BSW) required; Master’s preferred

• Minimum 1 year of experience in public health or a related field

• Strong knowledge of community resources and service access

• Effective communication and documentation skills

• Ability to work independently and as part of a multidisciplinary team

• CPR certification and Mandatory Reporter training (or within 30 days of hire)

• Valid driver’s license, insurance, and reliable transportation required

• Must pass physical exam, drug screening, TB test, and background check

Working Conditions:

• Medium physical activity (up to 50 lbs occasionally)

• Exposure to environmental and health-related hazards

• Flexibility in scheduling and regular travel between locations required
