What you’ll do

• Deliver skilled PTA interventions under the plan of care set by the PT (strength, balance, gait, transfers, HEP).
  
• Educate patients/caregivers and tailor home programs that actually get used.
  
• Document clearly and on time in our EMR (same-day notes expected).
  
• Communicate changes in condition and progress to the PT and care team; help prevent avoidable hospitalizations.
  
• Support safe transitions of care and stellar patient experience (HHCAHPS pride!).

• On-time starts, on-time notes, and measurable functional gains.
  
• Patients/caregivers who feel seen, supported, and confident with their HEP.
  
• Zero surprises—tight communication and closed loops with your PT and nurse partners.

Requirements:

What you’ll need

• Active Oklahoma PTA license & BLS/CPR
  
• Valid driver’s license, reliable vehicle, auto insurance
  
• Home health experience preferred (not required); strong new grads welcome
  
• Excellent communication, documentation, and follow-through
  
• Comfort teaching safe mobility/HEP in real-world home environments
  

Schedule

• Monday–Friday with occasional flexibility; we build a route that makes sense for patients and for you

HH and HCHB experience is a plus
