About the Role:
The Home Health Physical Therapist plays a crucial role in enhancing the quality of life for patients recovering from injuries or surgeries in the comfort of their own homes. This position involves developing personalized rehabilitation plans that address the unique needs of each patient, ensuring they regain mobility and strength effectively. The therapist will work closely with patients, their families, and other healthcare professionals to monitor progress and adjust treatment plans as necessary. By providing hands-on therapy and education, the therapist empowers patients to take an active role in their recovery journey. Ultimately, the goal is to facilitate independence and improve overall physical function, allowing patients to return to their daily activities with confidence.
Minimum Qualifications:
- Valid state license to practice as a Physical Therapist.
- Masters or Doctor of Physical Therapy (DPT) degree from an accredited program.
- Experience in orthopedic rehabilitation and manual therapy techniques.
- Current CPR Card
Preferred Qualifications:
- Experience working in a home health setting.
- Bilingual skills to communicate effectively with a diverse patient population.
- Strong Documentation Skills to support PT needs
Responsibilities:
- Conduct comprehensive assessments of patients' physical abilities and limitations.
- Develop and implement individualized treatment plans based on assessment findings.
- Provide hands-on physical therapy techniques, including manual therapy and therapeutic exercises.
- Educate patients and their families on exercises, safety measures, and strategies to enhance recovery.
- Monitor and document patient progress, adjusting treatment plans as needed to ensure optimal outcomes.
- Identify and meet patients’ goals and needs
- Offer cost-effective treatments that help improve clients’ motion and mobility
- Consult and practice with other health professionals
- Evaluate effects and monitor and communicate progress
- Document patient care services
- Supervise Physical Therapy Assistants/Aids

The required skills in physical therapy and rehabilitation are essential for assessing and treating patients effectively. Daily work involves applying orthopedic rehabilitation techniques to help patients recover from injuries, ensuring they regain strength and mobility.
