- Full-time: The position offers a full-time schedule, Monday through Friday, with occasional potential for weekend and evening work.
Responsibilities:
- Conducting initial and ongoing assessments of clients to determine their care needs.
- Developing, implementing, and updating client care plans in collaboration with physicians and the home health team.
- Providing direct skilled nursing care to clients of varying ages and acuity levels.
- Coordinating care with families, caregivers, and other healthcare providers.
- Medication Management: Ensuring proper medication administration and reconciliation.
- Client Monitoring: Regularly monitoring clients conditions and reporting any changes to the appropriate personnel.
- Providing education and support to clients, families, and caregivers.
- Ensuring accurate and timely documentation of client care and progress.
- Ensuring the safety needs of clients are addressed through home environment assessments and individualized abuse prevention plans.
- Providing clinical oversight, delegation, and leadership to other licensed and unlicensed personnel.
- Documentation: Maintaining accurate and up-to-date client records.
- Compliance: Adhering to all relevant regulations, including OSHA and HIPAA.
- Managing your schedule effectively and participating in the on-call phone rotation on occasion.
- Currently licensed and unrestricted as a Registered Nurse (RN) in the State of Minnesota and Wisconsin.
- Certified in CPR and have a negative TB skin test or chest x-ray.
- Minimum of one (1) years of experience in a health care facility or home care setting. Home Health Experience: Two or more years of clinical experience, with one year in a home health agency, primary care clinic, or health facility, is preferred.
- Driving: A valid driver's license and reliable transportation are necessary.
- Demonstrates critical thinking skills and the ability to make appropriate clinical judgments.
- Excellent written and oral communication skills.
- Proficiency in using computer systems and electronic charting is essential.
