Nationwide Therapy Group is seeking a LPN / LVN for a travel job in Utica, New York.

Job Description & Requirements

• Specialty: LPN / LVN
• Discipline: LPN / LVN
• Duration: 13 weeks
• 40 hours per week
• Shift: 8 hours, evenings, nights
• Employment Type: Travel

• Provide routine nursing care under the supervision of an RN or provider.
• Administer medications and treatments as prescribed.
• Monitor residents for changes in condition and report to RN or physician.
• Support residents with ADLs and ensure their comfort and safety.
• Complete documentation in PointClick Care (PCC) EMR.
• Participate in care planning and team meetings.
• Follow infection control and patient safety procedures.
• Assist in managing dementia and memory care behaviors appropriately.
• Float across multiple units within the facility when required.
• Patient Population:
• Dementia and memory care
• Multiple sclerosis
• Orthopedic recovery
• General geriatric patients

Active NY LPN license 1+ years of LPN experience in LTC or similar setting BLS certification
