We are seeking a dedicated Community Care Provider to join our team. Reporting to the Clinical Manager, you will be responsible for providing strength-based, solution-focused therapeutic services in the community. Our company prides itself on delivering consumer-driven and family-oriented services, conducted within the context of the consumers’ natural environment. We are looking for a candidate who is knowledgeable, adaptable, and committed to providing the highest quality of care to our clients.
Responsibilities- Provide strength-based, solution-focused supportive therapeutic services in the community.
- Deliver client-centered, individual Psychosocial Skills Rehabilitation (PSR) services in the community.
- Assess each recipient for Strengths, Resources, and Goals within first 30 days of treatment; provide Strength-based planning and treatment on an ongoing basis.
- Participate in collaborative meetings with Rehabilitation Services staff, other agencies or community and natural supports as needed.
- Provide Crisis Intervention services to recipients as needed according to agency policies, procedures, and service definitions.
- Communicate client’s status on a regular basis, including changes in mood or functioning, medical issues and all crisis and health and safety issues.
- Produce effective, clinical documentation of every contact with recipient.
- Assist with the completion of 90 Day client reviews
- Attend all office trainings as well as regular clinical supervision sessions with Clinical Manager
- Return phone calls promptly and follow all safety rules.
- Complete all yearly trainings
- Master’s Degree from an accredited university or college in the field of Counseling, Social Work, Psychology or Sociology.
- Ability to use computer interface to complete documentation for billing or other purposes as needed.
- Excellent communication and documentation skills.
- Ability to work collaboratively with a diverse team.
