The Ideal Candidate has a minimum of 1 year of hospice or home health experience.

The candidate must be comfortable driving from patient to patient, possibly in rural areas with windy roads. Must be comfortable in all home situations, including homes with pets.

MINIMUM QUALIFICATIONS:
  • At least 18 years of age
  • High school diploma or equivalent
  • Graduated from an accredited Home Health Aide program
  • Current CHHA license
  • 1 year of Home Healthcare / Hospice experience is preferred
  • Current CPR card for Healthcare Providers
  • Strong organizational and communication skills
  • Ability to travel per job requirement
  • Current driver's license with an automobile that is insured in accordance with state and organization requirements and is in good working order

Job Summary
We are looking for a Hospice Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

RESPONSIBILITIES:
  • This position is Per Diem
  • Provides personal care services to patients
  • Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning and doing household chores
  • Ensure high quality patient experience and positive health outcomes by providing hands-on personal care and assistance with activities of daily living (bathing, dressing, toileting, feeding, grooming, etc.)
  • Provides personal care including, baths, back rubs, oral hygiene, shampoos, and changing bed linen
  • Reports on patient's condition and significant changes to the assigned nurse. If unable to perform a certain task, reports to the primary nurse
  • Other duties as assigned
