Job Description
The Community Health Worker is responsible for helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW will build trusting relationships with clients and to connect those clients to care, eliminating barriers and advocating for systemic changes along the way. The CHW supports providers and the Case Managers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of clients and their families. The CHW will work to guide clients facing the effects of poverty, and/or experiencing homelessness, toward more permanent primary care services outside the hospital system. Establishing a secure, trusted connection with a primary care provider can help prevent persons facing poverty and/or homelessness from reliance on Emergency Departments to meet primary care needs. The CHW will be assigned to assist clients seeking assistance, to help them sign up for Medi-Cal, and provide linkages to medical care, and health education opportunities, with the
support of the CalAIM Services Manager.
Skills, Abilities, and Knowledge
- Good organizational skills to handle multiple priorities while remaining professional and calm
- Strong level of confidentiality due to the sensitivity of materials and information handled
- Ability to make suggestions on workflow or system efficiency and effectiveness
- Ability to work independently and be self-directed and flexible.
- Ability to work at a high-volume level of accuracy
- Experience working with vulnerable populations, or those facing a crisis
- Experience in assisting others with an application process
- Ability to instill a sense of calmness and trust
- Must be computer literate with proficiency using the internet, MS Office
- Programs (Power Point, Word, Excel), Google Drive, Adobe Acrobat Pro, Zoom, MS Teams
- Good problem-solving skills and the ability to prioritize multiple tasks
- Understanding of and sensitivity to the needs of unhoused and housing insecure individuals
- Experience in an environment serving vulnerable populations with physical and emotional health risks or the equivalent
- Strong interpersonal skills and ability to work effectively with staff, and professionals of diverse backgrounds
- Ability to work independently and as a member of a team
- Current driver’s license and clean driving record High School Diploma or equivalent
- Bilingual English/Spanish—verbal and written (Required)
- Experience working in a medical-related client service role is preferred
- Possession of a CHW certification of completion from a formal training program is preferred.
- Medical terminology and/or background preferred
Physical Requirements
- While performing the duties of this job, the employee is regularly required to sit for prolonged periods of time, talk, and hear.
- The employee is occasionally required to stand, walk, and use hands to operate a standard computer keyboard.
- The employee may occasionally lift and/or move up to 25 pounds.
