Yoursquo;ll help clients set goals, connect with resources, and maintain stable housing. Yoursquo;ll also screen individuals and families seeking affordable housing options and guide them through the process with compassion and care./liliAssess individuals and families in housing readiness, needs, and strengths/liliProvide weekly case-management services to assigned clients in the field, including working with clients to determine client support plan, goals, resources, and maintaining client case notes, history, and data through HMIS (Unity), utilizing best practices for keeping client information confidential./liliMay attend CoC and other community meetings, as well as present Ibis programs to community stakeholdersnbsp;/liliOversees data quality of all client files upon entry and exit from the program./liliCoordinates psycho-educational groups.nbsp;/liliMonitors monthly coordinated entry follow-ups and verifies all updates are made within the first week of each month./liliParticipate in staff and group meetings. May organize and run treatment and community meetings/liliMaintains client case notes in our EMR, history, and data, utilizing best practices to keep client information confidential. Generates and compiles client outcomes and reports within Clarity for reporting to funders for the program. Keep confidential records according to the Code of Ethics./liliMaintain minimum weekly contact to track client progress./liliMaintain professional boundaries./liliMay provide supervision to Interns (based on experience), ensuring that proper case management guidelines are being followed./liliVisit and meet with community service providers, service organizations, and institutions that prove beneficial to the maintenance of self-sufficient living./liliMeet personal goals and actively contribute to the achievement of team and organizational targets/goals. Attend required staff meetings and ongoing training.
