pMultiple areas utilize staff, including, but not limited to, the Inpatient Unit (IPU), Care 24 field, Care24 Triage and Continuous Home Care (CHC). In all roles, the RN utilizes the nursing process of assessment, planning, intervention, implementation and evaluation. The RN is skilled at assessment and management of pain and other complex symptoms frequently present at the end of life, including education and support to patient and family. The RN works with the interdisciplinary hospice team to plan and direct services to meet the needs of individuals and families./p

liAll Hospice RNrsquo;s will cross train to other departments to meet the needs of the organization./li
liProvide nursing services, treatments, diagnostic and preventive procedures requiring the skills of a registerednbsp;nurse according to policies, procedures and protocols./li
liParticipate in interdisciplinary group meetings as needed./li
liAssists in development, implementation and revision of the interdisciplinary team care plan for each patient and family. Establishes goals, interventions including the patient and family in the process./li
liAssess patient and report to the attending physician and appropriate interdisciplinary team members, response to treatments and medications as well as changes in the patientrsquo;s physical or emotional condition./li
liCoordinate care with other interdisciplinary team members and with community resources as needed./li
liTeach, supervise and counsel the patient/family regarding pain and symptom management and psycho-social aspects of care for the terminally ill./li
liAssess and maintain patient safety./li
liTeach the patient and family/caregiver self-care techniques as appropriate. Provide medication, diet and othernbsp;instructions as ordered by the physician and recognize and utilize opportunities for teaching identified needs with patients and families/caregivers./li
liAssist the patient and family/caregiver and other team members in providing continuity of care./li
liSupervise care provided by LPNrsquo;s and Home Health Aides./li
liAttend and participate in clinical education, in-service and nursing staff meetings./li
liApplies working knowledge of state, federal local and accreditation regulations for the delivery of hospice services./li
liDocuments accurate assessment of patient status and updates the plan of care as needed in the EMR./li
liDocumentation is completed at point of care and reflects interventions and response to care, focusing on patientrsquo;s goals./li
liMeets productivity expectations as set by the organization./li
liOther duties as assigned./li

pAll staff are expected to participate as needed in variable shifts, including weekend rotation per staffing needs, departmental holiday rotation and follow the Comprehensive Emergency Management Grid during times of activated emergency situations./p

h4Qualifications/h4brpstrongComputer Skills: /strongStrong computer skills including database software and general knowledge of email/outlook. Must be able to utilize device to document in Electronic Medical Record (EMR)./p
pstrongSupervisory Responsibilities:/strong This job supervises care provided by LPN and HHA’s./p
pstrongEducation/Experience:/strong Associates Degree in Nursing, Bachelor in Nursing (BSN) degree preferred. CHPN preferred. Hospice experience preferred. /p
pstrongLanguage Skills: /strongCommunication skills of reading, writing, and speaking in English./p
pstrongMathematical Skills: /strongMath skills needed including addition, subtraction, multiplication, division and percentages/fractions with ability to calculate medication doses. Nurse must successfully pass the math test given at time of interview prior to starting orientation and annually./p
pstrongCertificates, Licenses, Registrations: /strongCurrent Registered Nurse License to practice in the state of Florida Healthcare Provider CPR required upon hire and maintained. CHPN preferred; Required within 2 years of employment. Valid Florida Driver’s License required./p
