This is a unique collaboration between Hillside’s Preventive and Permanency Services, where extended relatives and fictive kin will be sought and gathered by a Permanency Specialist, and a carefully planned transfer will occur to a General Prevent Clinician, who will continue providing services to the youth and family. nbsp; nbsp; The Permanency Specialist is responsible for individualized permanency planning, which includes the delivery of intensive short-term Family Finding services.
