Position Overview:

Provide direct medical care under physician supervision, perform assessments, order tests, and manage treatments for residents in assigned units.

Key Responsibilities:

• Conduct physical assessments and develop care plans.
• Order and interpret diagnostic tests.
• Prescribe medications within the scope of practice.
• Document all care in EMR and maintain accurate records.
• Collaborate with physicians and nursing staff to ensure continuity of care.
• Participate in quality improvement initiatives and case reviews.

Requirements

Qualifications:

• Education:
• NP: Master’s in Nursing (or)
• PA: Accredited PA Program
• Licensure:
• Active NYS NP or PA License (good standing)
• Certification in a relevant specialty area preferred
• Experience: Post-acute/LTC setting (preferred)
• Technical Skills:
• Adhere to HIPAA and Medical Staff By-Laws.
• Ensure timely completion of documentation.
• Maintain payer credentialing and MCH approval.
• Soft Skills: Collaborative practice, clinical judgment, patient advocacy
