Weekend Requirements: Rotating weekends; holiday coverage as needed

Job Setting: Home Health

Responsibilities:

• Provide direct physical therapy services to home health clients.

• Collaborate with interdisciplinary team members in developing and implementing individualized care plans.

• Utilize rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, ADL equipment, and therapeutic modalities.

• Monitor patient progress and adjust interventions as appropriate.

• Maintain accurate documentation in compliance with agency and regulatory standards.

Requirements:

• Home Health Certified Physical Therapist

• 2+ years combined acute care (hospital-based) and home care experience; acute care experience preferred

• BLS (AHA)
