Job Description & Requirements

Licensed Practical Nurse (LPN) – Long-Term Care
Shift: 7:00 AM – 7:00 PM (12-hour days)
Schedule: 4x12s | 48 hours/week
Overtime: OT is billable after 48 hours/week
Contract Length: 91 Weeks
Guaranteed Hours: 48 hours/week
Facility & Unit Details:
- Setting: Long-Term Care (LTC)
- Census: ~105 residents
- Patient Ratios:
- Days: 4 nurses
- Nights: 3 nurses
- Staff evenly split across 4 halls based on acuity
- Staffing Model: Equitably distributed assignments per hall
Requirements:
- Experience:
- New travelers accepted with minimum 1 year prior nursing experience
- LTC experience strongly preferred
- Certifications:
- IV Certification required
- Licensure: Active LPN license in state of assignment
- Housing: Must secure own (no facility housing available)
- Transportation: Reliable transportation required
