Role Overview

The Physical Therapist (PT) accurately assesses, differentially diagnoses, and identifies deficits in a patient’s mobility and functional abilities. They develop individualized, patient-centered treatment plans to reduce pain, restore function, prevent disability, and maintain or improve mobility.

Essential Functions & Responsibilities:

• Perform patient evaluations, including history, systems review, and tests/measures to collect data.

• Establish individualized plans of care and select treatment techniques aligned with patient impairments and physician orders.

• Conduct ongoing reassessments and adjust treatment plans as needed.

• Initiate and manage discharge planning from the first visit through treatment completion.

• Supervise and delegate to ancillary staff and students in accordance with state laws and facility policy.

• Complete accurate, legible documentation per departmental, accreditation, and insurance requirements.

• Communicate with patients, families, caregivers, and the healthcare team to optimize care outcomes using AIDET principles.

• Assess and address patient, family, or caregiver educational needs.

• Participate in professional growth through in-services, workshops, and relevant literature.

• Engage in quality improvement initiatives.

• Maintain a safe environment for patients, visitors, and staff and comply with approved safety standards.

• Function within the scope of practice as defined by the state’s practice act.

• Perform other duties as required.

Submission Requirements

• 2 years PT experience in LTC setting – Required

• TX State License or Compact License – Required
