About Upward Bound House (UBH)
Upward Bound House is dedicated to ending family homelessness in Los Angeles. With over 30 years of experience, we provide a continuum of housing and support services that help families and young parents transition from crisis to stability. As we launch our new DMH-funded interim housing program, were seeking passionate, experienced professionals to join our growing team.
Overview:
Upward Bound Houses (UBH) Intensive Case Management Services (ICMS) program supports individuals experiencing chronic homelessness, medical conditions, and co-occurring disorders through long-term services referred by LA Countys Department of Health Services (DHS). The Program Manager oversees day-to-day operations, supervises staff, and ensures high-quality service delivery using a hands-on, lead-by-example approach.
Key Responsibilities:
Case Management (10%)
- Carry a small caseload of complex clients to support housing retention and stability.
- Supervise and guide case management staff, providing coaching and support with complex cases.
Program Oversight & Leadership (35%)
- Ensure program goals, metrics, and participant outcomes are met.
- Lead staff meetings, conduct performance reviews, and provide regular coaching and feedback.
- Collaborate on policies, training, and quality assurance with leadership.
Community & Outreach (10%)
- Represent UBH at community meetings and events.
- Collaborate with partners to identify and connect eligible participants to ICMS services.
Compliance & Administration (30%)
- Monitor program documentation in CHAMP and digital files; support audits and funder reviews.
- Complete reports for DHS, UBH, and finance teams; ensure accurate data tracking and submission.
Program Development & Evaluation (15%)
- Analyze data to track performance and improve outcomes.
- Support expansion initiatives and address system-level barriers with funders.
- Align program operations with HFH/HSH goals and contract requirements.
Qualifications:
- Masters Degree in Social Work or related field (MSW/MFT preferred).
- 24 years of leadership experience in social services.
- Proven success managing programs, supervising staff, and working with funders.
- Excellent communication, organization, and problem-solving skills.
- Ability to work in diverse, high-pressure environments and resolve conflicts effectively.
- Field-based ICMS experience strongly preferred.
Additional Requirements:
- Knowledge of homeless services, housing programs (e.g., Vouchers, Scattered Sites), and mental health systems.
- Familiarity with CHAMP, HMIS, DHS documentation, and Excel reporting.
- Ability to work flexible hours, including evenings/weekends.
- CPR/First Aid and required LAHSA/DHS training certifications (or willingness to obtain).
- Reliable transportation, a valid drivers license, insurance, and vehicle ownership.
- TB clearance and background check required.
