Indeed provides their own estimated salary calculator and is not affiliated with COH's range./strong/em nbsp;/ppbr//ppstrongHighlighted Duties and Responsibilities:/strong/ppstrongPatient Care:/strong/pulliWorks as part of an integrated care team at the Virginia Williams Family Resource Center to connect people who are experiencing homelessness and are pregnant to perinatal care and support systems.nbsp;/liliDuring initial intake, assesses client's connections or barriers to receiving perinatal services, preferences for care, risk factors, and works to opt in client to HONEY perinatal care coordination.nbsp;/liliIdentifies and links participants to appropriate and preferred community resources, perinatal care, and wraparound supports for family, in coordination with assigned housing case manager.nbsp;/liliOpts in, schedules, and sends rides for patients through Rides for Moms partnership with Uber Health (~ 30 rides per month)./liliDocument any relevant patient encounters in eCW within 3 business days of interaction. nbsp;Updates referral and client tracking databases within 1 business day of interaction with clients nbsp;/liliSecures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.nbsp;/li/ulpstrongOutreach amp; Education/strong:nbsp;/pulliConducts outreach activities to develop relationships with HONEY program participants and partnerships with other government and community-based organizations.nbsp;/liliConducts outreach to homeless service and healthcare providers to build partnerships and opportunities for collaboration, training, and client support (e.g.: nbsp;referrals, programming updates, etc.)./liliAttends community events, tabling events, meetings, and conferences as requested to promote the HONEY program to potential clients and partners.nbsp;/liliEducates partners, clinicians and housing system staff on issues of perinatal health to support families during pregnancy and infancy.nbsp;/liliConducts program overviews and educational sessions virtually, in person, and asynchronously for healthcare and homeless service providers to better support their work with pregnant people experiencing homelessness (approximately 2-4 sessions per month)./liliConsults with COH Maternal amp; Child Health team and Housing team members in the development of presentations and training materials/liliCreates pre- and post-test tools to assess the effectiveness of educational sessions, modifying and creating new training resources based on pre- and post-test data/liliPerforms other duties as assigned by supervisor./li/ulpRequirements/pulliBachelor’s Degree Required.nbsp;/liliPrior instructional experience required; experience in adult education, social services, health care, or nursing preferred.nbsp;/liliExperience working with under resourced populations, including pregnant people experiencing homelessness preferred./liliStrong organizational, written, and verbal communication skills./liliStrong interpersonal skills and comfortability in conducting cold calls and outreach./liliBasic graphic design, marketing, and presentation skills required./liliMust have reliable transportation.nbsp;/liliIn relation to remote work versus on-site expectations, this position is classified as the following: nbsp;nbsp;strongHybrid/strong: Remote work and on-site work requirements are generally split roughly 60% on-site/40% remote. nbsp;nbsp;This position will be based primarily at Virginia Williams Family Resource Center (at 64 New York Avenue NE), with on-site expectations of three days a week, of which one day will be Monday.
