Nationwide Therapy Group is seeking a LPN / LVN for a travel job in Lancaster, New Hampshire.

Job Description & Requirements

Key Responsibilities:

• Provide direct patient care in accordance with the nursing plan of care and facility policies
• Assist in patient assessment and report significant changes to RN/physician
• Administer prescribed medications and treatments accurately and safely
• Monitor patient vital signs and document care in the medical record
• Provide wound care, catheter care, and other nursing procedures as assigned
• Educate patients and families on treatment plans, medications, and post-discharge instructions
• Maintain a clean and safe patient environment
• Collaborate with interdisciplinary team members to ensure quality patient outcomes

Requirements:

• Active and unrestricted LPN license in [NH or Compact]
• BLS certification (required)
• Minimum 1–2 years of LPN experience (acute care or long-term care preferred)
• Strong clinical and assessment skills
• Excellent communication, time management, and teamwork abilities
