Medi Home Hospice, a proud member of the Medical Services of America, Inc. family, currently seeks an experienced Full-Time Medical Social Worker to join our Hospice team to cover Fayetteville and Lumberton (Hoke, Robeson, Cumberland, Bladen, Scottdale, and Sampson), NC areas.

Responsibilities Include:

\n

· Provides social work services in accordance with the physician’s plan of treatment.

\n

· Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.

\n

· Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.

\n

· Serves as a liaison with other community resources.

\n

\n

Job Requirements

\n

· Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.

\n

· Minimum of one-year Hospice experience preferred.

\n

· Valid Driver's License.

\n
