Job Summary

Responsible for providing compassionate and comprehensive care to patients in a home health setting. Assess patient needs, develop care plans, and administer medications.

Essential Duties and Responsibilities

1. Professional Responsibilities:
2. Assess patient needs and render appropriate care.
3. Perform patient teaching, functions as a member of the health care team in delivering mission-driven, quality patient care.
4. Attend all in-services, education programs and scheduled meetings as outlined by the Director of Nursing/Home Health Director.
5. Consistently demonstrate proper nursing judgment, deliver patient care within the scope of service for RN.
6. Maintain and demonstrate constant awareness of State and Federal Regulations pertaining to Home Health care.

1. Core Duties:

1. Demonstrate performance standards
2. Appropriately evaluate signs and symptoms of disease processes, reactions to treatments, medication and effectiveness of teaching.
3. Deliver age-appropriate treatment according to the patient’s plan of care
4. Complete a thorough assessment of the patient’s pain and intervene appropriately
5. Communicate appropriately and respectfully the nursing process and treatment according to the patient/caregiver ability to comprehend
6. Develop and document appropriate goals
7. Follow agency policy regarding discharge planning: complete necessary discharge information (D/C Summary)
8. Communicate and document patient updates to physicians, other members of the health care team and to the patient’s medical record timely
9. Develop, implement and update nursing goal/patient outcomes and aide care plan as necessary.
10. Supervise home health aide to ensure quality patient care and update care plan as needed.
    1. Complete patient scheduling and notify patient/caretaker and staff of any changes in an appropriate and timely manner
    2. Appropriately evaluate signs and symptoms of disease processes, reactions to treatment, medications and effectiveness of teaching.
    3. Follow agency policy regarding discharge planning, complete necessary discharge information (D/C Summary/OASIS)
    4. Follow orders given by provider
    5. Perform other job duties and tasks assigned

QualificationsEducation and Experience

• Graduate of an accredited school of nursing
• Current Mississippi RN license
• CPR certification

Preferred Qualifications

• Prior Home Health and/or Med Surg floor experience

Minimum RequirementsEssential Skill Competencies:

• Body system assessment
• Phlebotomy: Establish IV access
• Safe blood administration
• Medication monitoring
• Preventive skin care
• Chest tube care
• Aseptic wound care/ dressing change
• Foley catheter insertion/care

Working Conditions

Ability to sit, bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
