• Schedule:Monday"Friday, 8am"5pm; weekends off; rotating on-call (phone only, a few nights/weekends per month)

Role Overview

• Provide comprehensive, value-based primary care to seniors in a PACE program

• Collaborate with an interdisciplinary team to deliver patient-centered care

• Focus on long-term, meaningful relationships with patients

Requirements

• Board certification in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics

• Eligible to provide services to Medicare and Medi-Cal participants
