Responsibilities:
- Deliver specialized wound care as a nurse practitioner in long-term care settings
- Utilize cutting-edge wound care technology and evidence-based protocols
- Conduct thorough assessments and develop personalized treatment plans that address underlying health issues and co-morbidities
- Offer a consultative, whole-patient approach in collaboration with facility teams and families
- Build lasting relationships based on trust, advocacy, and compassionate care
- Document findings accurately using secure electronic health systems
- Practice with independence while being supported by an experienced, multidisciplinary clinical team
Qualifications:
- Active Pennsylvania (PA) Nurse Practitioner license (board-eligible not accepted)
- DEA license
- At least 2 years of wound care experience
- Previous experience in SNFs, long-term care, or rehabilitation facilities preferred
- Willingness to travel to designated partner facilities in Cumberland County, Franklin County, and surrounding areas
