Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients./ppstrongRESPONSIBILITIES/strong/ppemReasonable accommodation may be made for individuals with disabilities to perform essential job functions./em/pulliProvide case management with crisis intervention, discharge plan, group amp; individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management./liliComplete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required./liliFacilitate placement including pre-placement visits, and transportation as needed. nbsp;/liliShift coverage including part of the on-call team./liliProvide field-based services as required by program and client need./liliMaintain confidentiality of clientrsquo;s care./liliParticipate in scheduled program meetings./liliOther relevant duties as assigned./li/ulpstrongQUALIFICATIONS/strong/pulliMaster#39;s degree; or bachelor#39;s degree with four years of experience in social services./liliFor SUD CM a certification from recognized board is preferred and registration required./liliFor Housing two years of experience and an AA degree in social service or related field or High School Diploma and 5 yearsrsquo; experience./liliValid CA driverrsquo;s license and current auto insurance per contract requirements required./liliExperience providing mental health and/or substance use disorder services preferred./liliCultural competency understanding required./liliBilingual preferred./liliKnowledge of Microsoft Office and EHR software preferred./li/ulpstrongPHYSICAL REQUIREMENTS/strong/ppThese physical demands represent what must be met to successfully perform the essential functions of this position.
