Some duties may include collecting patient information and data, provide patient care, communicate with patient, and care team member. May work in a team supporting multiple providers.br /br /Individuals in this role will work in an outpatient clinic setting.br /br /bPrinciple Responsibilities/b/pulliProvides patient care support as delegated by Provider or RN.ulliAdministers therapies including medications, scheduling follow up visits (and prior authorization) or other support unique to a specialty provider./liliProvides patient care pre/post encounter or procedure, preventative or other health information./liliCollaborates with Provider and team on improving and managing quality goals./liliMay start, monitor, and discontinue IVs (dependent on site needs and training)./li/ul/liliRooms patients, manages patient flow and obtains adequate patient medical information/data.ulliPerforms rooming and accurately measures patients’ vital signs and other measurement/indicators as necessary and records in medical record./liliCoordinates communication of medical information and confirms information with patient when necessary and document per practice. policy and standards/liliAssists patients, as necessary, to prepare for physician exam, including providing psycho-emotional support./liliAssists in the collection, labeling and processing specimens timely/accurately./liliAssists physician with procedures including preparation of medication as required/needed./liliOrganizes patient flow so physician can move efficiently from one room to the next./liliProvides outbound calls to patients to follow up as directed which may include notifying patients of test results./liliAccurately and completely charts information in the patient medical record./liliMay checkout and schedule follow up visits with provider or appropriate staff before patient leaves./li/ul/liliReceive or initiate patient phone calls.ulliAnswers phones cordially and professionally in a timely manner./liliMay confirm or reiterate patient symptom using patient#39;s own words./liliArrange for appropriate appointment or forward to RN or provider for review/recommendation./liliDocuments all encounters and patient interactions accurately and completely in the electronic medical record./li/ul/liliProcesses physician orders.ulliPerforms any clinical functions within scope of license as ordered by physician./liliResponds promptly to patient needs and requests./liliReviews patient charts before the visit to ensure that test results, therapy reports etc. are available./liliProvides appropriate instructions to patient as directed by physician./li/ul/liliCleans, prepares, organizes and re-stocks exam rooms.ulliCleans and ensures the proper maintenance occurs for all exam rooms and sterilize instruments as trained./liliEnsures that required teaching supplies are restocked and readily available./liliStocks and supplies exam rooms according to standards and patient needs./li/ul/liliOther duties as assigned./li/ulpbr /bRequired Qualifications/b/pulliAssociate#39;s or Vocational degree from a Board of Nursing approved LPN program/li/ulpbr /bPreferred Qualifications /b/pulliTwo years clinic LPN experience/li/ulpbr /bLicenses/Certifications/b/pulliLicensed Practical Nurse MN BON or Licensed Practical Nurse Wisconsin (if working in Wisconsin or interacting with Wisconsin patients remotely)/liliMay require both state licenses based on the services provided at the location hired/liliMust meet at least ONE of the requirements below:br /◦ Current BLS certification from the American Heart Associationbr /◦ Current BLS certification from the American Red Crossbr /◦ Allina in-house BLS training (within 30 days of hire)/li/ulpbr /bPhysical Demands/b/pulliLight Work:/liliLifting weight up to 20 lbs. occasionally, up to 10 lbs. frequently/li/ulp/pp style="text-align:left"bPay Range/b/ph3/h3p style="text-align:inherit"/pPay Range: $25.41 to $31.83 per hourh3/h3p style="text-align:inherit"/pThe pay described reflects the base hourly pay range.
