This position is a critical resource to help manage staff and promote teamwork, while meeting individual performance expectations and contribution to shared team performance goals. This position supports and strengthens the link between referral sources and THAH to deliver seamless care to the patient./ppThe Manager Customer Care Center monitors team performance and adherence to standards of care to ensure timely transitions to home based services from acute care and skilled nursing facilities to THAH. The Manager monitors volume and maintains staffing within the region and regularly provides performance feedback to staff by monitoring achievement towards goals.
