Heart Matters Home Health Care is a non-medical home health care agency serving the Northern Virginia area. We are dedicated to providing compassionate care for elderly clients, cancer patients, and individuals with disabilities. 

"In Home Caregiver" is a vital role within our organization located in Virginia.  This position is essential in delivering personalized support and companionship to our clients, ensuring their comfort and well-being while aligning with our mission of compassionate care.

We are seeking a Home Health Aid to become a part of our team! You will provide routine healthcare to patients at the patient's home or in a care facility.

Responsibilities:​​​​​​

• Provide routine individualized care to the elderly, convalescents, or persons with disabilities
• Offer companionship and emotional support to clients
• Monitor client health anand report changes in patient health status 
• Assist with medication reminders and adherence
• Maintain a clean and safe environment for clients
• Collaborate with family members and healthcare professionals to ensure comprehensive care
• Document client progress and any incidents during caregiving sessions
• Adhere to all safety protocols and company policies
• Provide personal care, such as bathing and dressing patients
• Provide specialized healthcare such as administering medication and changing bandages
• Teach family members ways to care for patient

Qualifications:

• Experience in hospice, home health, or other related field
• Provide experince in caregiving with elderly or disabled individuals
• Strong understanding of geriatric care principles
• Excellent communication skills and empathy towards clients
• Ability to work independently as well as part of a team
• CPR/First Aid certification preferred, but not required
• Flexibility to adapt to changing client needs
• Positive and patient d emeanor
• Ability to build rapport with patients
• Ability to handle physical workload
• Excellent written and verbal communication skills
