Position: Family Medicine / Geriatric Medicine Physician (Internal Medicine / Geriatric Medicine)

Position Overview:
We are seeking a dedicated and compassionate Physician specializing in Family Medicine or Internal Medicine with a focus on Geriatric Medicine to join our esteemed medical team in Ridgeland, South Carolina. This full-time, permanent position offers an excellent opportunity to provide comprehensive primary care to the geriatric population, enhancing quality of life and health outcomes for seniors in the community.

Key Responsibilities:

• Deliver comprehensive primary care services to patients aged 65 and older, including preventive care, chronic disease management, and acute illness treatment.
• Conduct thorough patient assessments, develop individualized care plans, and coordinate with multidisciplinary teams to optimize health outcomes.
• Emphasize patient-centered care, ensuring clear communication and empathetic patient interactions.
• Manage medication reconciliation and review, including addressing polypharmacy concerns common in geriatric populations.
• Stay current with advancements in Geriatric Medicine and integrate best practices into clinical care.
• Maintain accurate and timely documentation within electronic health records in compliance with healthcare regulations.
• Participate in quality improvement initiatives and contribute to the overall clinical excellence of the practice.

Qualifications:

• MD or DO degree from an accredited institution.
• Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
• Valid medical license to practice in South Carolina.
• Prior experience working with the elderly population is preferred but not mandatory; a strong interest in Geriatric Medicine is essential.
• Exceptional communication skills and a compassionate approach to patient care.
• Ability to work collaboratively within a multidisciplinary team.
