We are a well established multi-specialty practice seeking an intelligent, kind-hearted APN/PA for a distinctive Sub-Acute Rehabilitation facility and Hospital facility. Looking for a strong candidate to work with a Transitional Care Team in our affiliated Post-Acute facility in Bergen County. Job responsibilities include treating patients with complex medical problems with a goal to prevent readmissions and transition patients back to the community. Plenty of hands on teaching, training and support. Experience with hospital or post acute care highly desirable but a strong background in Med/Surgical nursing rotations and medical floor experience would suffice. Practitioner will be a part of a comprehensive care team. **Candidate must also be able to obtain full staff privileges at The Valley Hospital in Paramus, NJ. CANDIDATE MUST HOLD A VALID/CURRENT NEW JERSEY LICENSE**

Job Type:

- Full Time (Sub-Acute rehab center / 3 days per week: 2 weekdays + Saturday or Sunday and additional hospitalist work)

- Part Time (Sub-Acute rehab center on weekday/weekend)

*The days and schedule are flexible (Either 12-hour or 8-hour shifts)*

Pay Type:

Hourly Pay for 8-hour shifts or Part Time Position: $65.00 - $75.00 per hour

Full-Time Salary: $150,000.00 - $170,000.00 per year with Bonus opportunities

Licenses/Certifications Required:

• Board-Certification
• APN / PA License (NJ)
• DEA License
• CDS License

Clinical Skills Required:

• Strong clinical knowledge and expertise in a wide range of medical conditions and treatments.
• Proficiency in using all aspects of electronic medical record systems, such as eClinicalWorks.
• Excellent communication skills to effectively interact with patients, families, and other healthcare professionals.
• Ability to provide compassionate care while maintaining professionalism and using evidenced based guidelines.
