Part Time Nurse Practitioner (NP) Home Health Flexible Schedule - Oskaloosa, IA

Bachelor of Science in Nursing (BSN) degree, active Nurse Practitioner (NP) license and minimum 1+ years Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered.

There is an open Part Time Nurse Practitioner (NP) Home Health Flexible Schedule job in Oskaloosa, IA. The Part Time Nurse Practitioner (NP) will conduct home health risk assessments within Mahaska County, IA. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Oskaloosa, IAand the surrounding Mahaska County, IA.Mahaska County, IA includes locations like New Sharon and Eddyville.Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities:

• Conduct annual home health risk assessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, medical conditions and medication history
• Provide education on medical conditions and medications
• Identify any gaps in care and provide recommendations for resolution
• No creation of treatment plans or prescribing of medications is required

Work Hours:

• Flexible shifts between Monday and Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Preferred Experience:

• 1 year of current NP experience is preferred

Preferred Qualifications:

• Nurse Practitioner License in IA
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC

Education Requirements:

• MSN degree from an accredited school of nursing
