pDescription/ppstrongJOB SUMMARY:/strong This position includes a combination of care coordination, data management, and direct clinical duties for adults who have a history of substance use and who may still be actively using various substances. The Case Manager is responsible for screening individuals, doing a comprehensive needs assessment, connecting individuals with various resources, and providing basic employment skills for adults with Substance Use Disorder (SUD) through developing strong relationships with clients and community providers.nbsp;/ppstrongESSENTIAL FUNCTIONS:/strong/polliWorks closely with other staff as a member of a treatment team;/liliConducts comprehensive needs assessments for individuals;/liliAssists clients with accessing resources, benefits, and entitlement programs;/liliObtains consents, releases of information, and requests collateral information from appropriate sources;/liliDevelops treatment plans for substance-use disorders for assigned clients that is consistent with program and BCTS policies and procedures and applicable OAR's;/liliConducts client Urine Analyses as needed and appropriate;/liliFollows all program policies and procedures for client case management and recordkeeping;/liliResponsible for recording information in the client files assigned to them and consistent with those policies and procedures and applicable OAR’s;/liliProvides support to SUD clients in achieving their treatment plan (ISSP/Individual Services amp; Support Plan) goals and objectives;/liliDevelops and maintains strong collaborative relationships with a wide range of community partners, including (but not limited to): Self-Sufficiency and Seniors/People with Disabilities, the County Adult Community Justice, Public Health, Mosaic Medical and other primary care clinics including dental;/liliAssists clients who have been identified to be in the ACT-Assertive Community Treatment nbsp;program with completing their Personal Recovery Plan, under the direction of the clinical supervisor;nbsp;/liliProvides transportation for clients when appropriate and under the direction of the clinical supervisor;nbsp;/liliStays abreast of community resources that individuals may be eligible for, including housing resources and housing options;/liliReceives training to become an OHP assister, to help individuals enroll in Oregon Health Plan if eligible;/liliAssist clients in determining if they are eligible for expungement of criminal record;/liliProvides transportation for individuals when appropriate and under the direction of the Supervisor;/liliFacilitates psychoeducational and skills groups (if appropriate);/liliMaintains clinical documentation in a timely manner in client files in the EHR system and in compliance with BestCare timelines and regulatory standards;/liliModels appropriate counselor behavior to the newer staff, and helps train and orient new staff to the program;nbsp;/liliIn addition to being a positive role model for staff, also provides assistance to clients, demonstrating compassion and being effective in engaging clients in mental health treatment services wherever possible;/liliOther related duties as assigned./li/olpstrongORGANIZATIONAL RESPONSIBILITIES:/strong/polliPerforms work in alignment with the BestCare’s mission, vision, values;/liliSupports the organization’s commitment to diversity, equity, and inclusion by fostering a culture of open-mindedness, cultural awareness, compassion, and respect for all individuals;nbsp;/liliStrives to meet Program/Department goals and supports the organization’s strategic goals;/liliAdheres to the organization’s Code of Conduct, Business Ethics, Employee Handbook, and all other relevant policies, procedures, and other compliance needs;/liliUnderstands and maintains professionalism and confidentiality per HIPAA, 42 CFR, and Oregon Statutes;/liliAttends required program/staff meetings and completes assigned training timely and satisfactorily;/liliEnsures that any required certifications and/or licenses are kept current and renewed timely;/liliWorks independently as well as participates as a positive, collaborative team member./liliPerforms other organizational duties as needed;/li/olpstrongREQUIRED COMPETENCIES:/strong Must have demonstrated competency or ability to attain competency for each of the following within a reasonable period:nbsp;/polliDemonstrated knowledge and ability to provide treatment of substance-related disorders including client evaluation, ASAM, and individual, group, family, and other counseling techniques;
