pDescription/ppstrongJOB SUMMARY:/strong This position includes a combination of care coordination, data management, and direct clinical duties for adults who have a history of substance use and who may still be actively using various substances. The Case Manager is responsible for screening individuals, doing a comprehensive needs assessment, connecting individuals with various resources, and providing basic employment skills for adults with Substance Use Disorder (SUD) through developing strong relationships with clients and community providers.nbsp;/ppstrongESSENTIAL FUNCTIONS:/strong/polliWorks closely with other staff as a member of a treatment team;/liliConducts comprehensive needs assessments for individuals;/liliAssists clients with accessing resources, benefits, and entitlement programs;/liliObtains consents, releases of information, and requests collateral information from appropriate sources;/liliDevelops treatment plans for substance-use disorders for assigned clients that is consistent with program and BCTS policies and procedures and applicable OAR's;/liliConducts client Urine Analyses as needed and appropriate;/liliFollows all program policies and procedures for client case management and recordkeeping;/liliResponsible for recording information in the client files assigned to them and consistent with those policies and procedures and applicable OAR’s;/liliProvides support to SUD clients in achieving their treatment plan (ISSP/Individual Services amp;
