pDescription/ppstrongJob Summary/strong:/pp· The social worker or therapist will complete a comprehensive emPsychosocial History/Assessment/em within three days./pp· The social worker or therapist is responsible for attending and assisting psychiatrist/APN in facilitating Interdisciplinary Treatment Team Meetings./pp· The social worker or therapist will make recommendations and address family and community resource issues in treatment team meetings./pp· The social worker or therapist will be responsible for ensuring that discharge planning is started at the time of admission and documented in the emInterdisciplinary Progress Note/em. Updates to Discharge Planning will be made throughout the treatment process as indicated and documented in the emInterdisciplinary Progress Notes. (/emSee Discharge Planning policy 205.1)/pp· The social worker or therapist will be actively involved in the direct care of the patient, providing individual, group, family therapy and teaching Psychotherapy session, if needed. Each intervention will be documented on the day of treatment according to emMedical Record/em emDocumentation /emPolicy (Policy 220.0)/pp· The social worker or therapist, under direction of Psychiatrist/APR, is responsible for the clinical decisions involving the patient’s psychiatric illness./pp· The social worker or therapist may be responsible for contacting outside referral sources to inform them of the referred patient’s admission, progress, and discharge, with consent of patient./pp· The social worker or therapist will be responsible for documenting a emMaster Treatment Plan/em, emTreatment Plan Review/em as indicated./ppRequirements/ppstrongEducation/Licensure Requirements/strong:/pp· Licensed master’s in social work (LMSW)--- master’s degree in social work/pp· Licensed clinical social worker (LCSW)---- doctoral degree in social work/pp· Licensed professional counselor (LPC) --- master’s in counseling/ppbr//ppstrongPhysical Requirements/strong:/pp· Minimum of standing for 1 hour per day/pp· Minimum of sitting for 2 hours per day/pp· Minimum of walking for ½ hour per day/pp· Have good manual dexterity/pp· Have good eye-hand-foot coordination/pp· Have good clarity of vision/pp· Bending, squatting, kneeling, balancing, pulling with force, reaching above head, reaching above shoulder, twisting at waist, pull/push up to 5 pounds, lift/carry up to 15 pounds, lift from floor level up, lift from waist level up, and/or lift above shoulders/head/ppbr//ppstrongExperience Requirements/strong:/pp· Two to three years of clinical experience with older persons, and hospital or nursing home experience preferred./ppbr//ppstrongJob Responsibilities/strong:/pulliConduct three group sessions daily, individual and telehealth sessions when needed/liliPlan topics for group sessions/liliDocument patients therapy notes daily/liliDocument and update monthly treatment plans/liliComplete a psychosocial assessment on new patients/liliComplete a master treatment plan on new patients/liliMake recommendations and address family amp; community issues that arise/liliMaintain open communication between members of the treatment team (physician, nurse, other therapist, CNA/transporters)/liliAbility to manage caseload and documentation expectations consistently/liliStrong communication and interpersonal skills for effective patient interaction and team support/liliAbility to maintain confidentiality and adhere to ethical guidelines/liliAbility to function within and contribute to a team environment/li/ulpThis job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee.
