Palliative Care Physician - Covina, CA

Job Description:

We are seeking a compassionate and board-certified Palliative Care Physician to join our interdisciplinary team. This role offers the flexibility to set your own pace with rounds, supported by a dedicated Nurse Practitioner (NP) to assist in managing patient caseloads. You'll provide specialized palliative care to patients in LTAC/STAC and acute care settings, collaborating closely with an interdisciplinary team to offer patient-centered care plans and manage complex symptoms.

Key Responsibilities:

• Lead patient care in LTAC/STAC and acute care facilities, supporting clinical decision-making and guiding patients and families through complex medical situations.

• Manage serious illness symptoms, including prescribing medications, and monitor treatment effectiveness in collaboration with the NP.

• Oversee and direct the development of proactive care plans, ensuring patient safety and optimal outcomes.

• Conduct rounds with the support of a Nurse Practitioner (NP) who will assist with patient visits, documentation, and follow-ups.

Qualifications:

• Doctor of Medicine (M.D.) from an accredited institution

• Board Certified or Board Eligible in Palliative Care

• Active DEA License and CA License

• Proven experience in LTAC/STAC settings preferred

• Strong interpersonal and communication skills to connect with patients, families, and healthcare teams
