Supervisory Report: Nurse Manager
Wage Classification: Non Exempt
Job Summary:
The Licensed Practical Nurse is responsible for clinical activities involving routine office nursing care and patient flow, patient recordkeeping and follow-up, patient education, and other related activities as required. The L.P.N. functions under the supervision of a Physician or Registered Nurse.
Essential Responsibilities:
The following duties are not intended to serve as a comprehensive list of all duties performed by all associates in this classification. Incumbent(s) may not be required to perform all duties listed and may be required to perform additional, position-specific duties.
- Routine nursing duties including preparing patients for examination: check vital signs, weight, and height. Perform standard clinical and laboratory procedures, Accuchek and other screenings as required in accordance with established clinical protocols.
- Administers patient injections and vaccines as directed by the clinical providers.
- Performs routine vision screening as appropriate.
- Practices safety, environmental, and/or infection control methods.
- Cleans exam rooms after each visit and at end of day.
- Update patient medication list in patient EMR every visit.
- Complete past medical, social, surgical, and family history on every new patient. Documents patient assessment and intervention data using established EMR systems and documentation practices.
- Update immunization records for every child visit; research immunization records in state database as required.
- Complete child physical forms and other physical forms as required by outside agencies
- Complete pregnancy visit paperwork in advance (complete pregnancy test, certification of positive pregnancy test, prescriptions)
- Keep rooms stocked with requisite medical supplies, table paper, etc.
- Prepare equipment and ensure that medical equipment is in good working order.
- Completes growth chart for every child well visit.
- Perform triage and telephone response to patients as assigned. Consult with physician, registered nurse, other health providers, and community resources to prevent/resolve problems and concerns.
- Call patients with lab results, and other reports as requested by provider.
- Call in medications to pharmacies.
- Participates in multidisciplinary teams to improve patient care processes and outcomes.
- Schedule appointments for patients as needed.
- Educate patients and/or families about health status, health maintenance and management of acute and chronic conditions.
- Assist indigent patients by completing pharmaceutical applications where applicable
- Other duties may be assigned as necessary.

Education and/or Experience:
L.P.N. must be a graduate of a licensed Practical Nursing Program and hold a current Delaware L.P.N. license.
Language:
Bilingual (English/Spanish) skills preferred.
Performance Requirements:
Knowledge:
- Knowledge of nursing practices.
- Understanding of standard use and care of medical equipment and supplies.
- Knowledge of EMR system, medical record documentation requirements and state/federal laws related to the release of health care information.
Skills:
- Skill in solving practical problems.
Abilities:
- Ability to accept, interpret and implement orders from the qualified professionals on his/her team.
- Ability to communicate effectively and develop rapport with patients and families to facilitate implementation of a care plan.
- Ability to calculate and administer drug dosages and injections.
- Ability to manage time
- Ability to organize and supervise nursing care of patients in an office setting;
- Ability to maintain and interpret records and reports
- Ability to act quickly and calmly in emergency situations.
Equipment Operated:
Wide range of standard medical equipment including syringes, stethoscope, scales, wheelchairs, thermometers, etc. Computer use and proficiency required.
Work Environment:
Medical exam/treatment rooms and medical offices in Health Center and remote sites. Controlled lighting, air quality, and temperature settings. Environmental hazards may be unpredictable including exposure to communicable diseases and biohazards.
Mental/Physical Requirements:
Involves standing, walking, kneeling, bending, grasping, manipulating and squatting and lifting (up to 50 pounds). Walking or standing may be more than four (4) hours per day. Must be able to work in a standard medical office setting including use of a computer, and standard office equipment. Must have ability to read computer screens, printed materials and hearing and speech to communicate in person as well as over the phone. Must be able to attend meetings or make presentations at various sites when required. Stress can be a component of this job, triggered by an individual’s response to workload or specific cases.
