Pediatric Nurse Practitioner or Physician Assistant - Shaping the Future of Pediatric Care Join Our Dynamic Team at Legacy Community Health

Based in the vibrant community of Baytown, TX, Legacy Community Health is seeking an innovative Pediatric Nurse Practitioner or Physician Assistant with at least two years of experience. Here, your contributions will not only deliver comprehensive, quality, and affordable healthcare to the community but also play a pivotal role in our ever-evolving healthcare journey. Core Responsibilities
  • Initiate thorough assessments of patients' health, adapting from focused to comprehensive evaluations based on the care context.
  • Collaborate with the Service Line Director and Clinic Site Director to continually enhance patient services and contribute to clinic development initiatives.
  • Consult with interdisciplinary teams including supervising Physicians and specialists to ensure the highest standard of patient care.
  • Document patient interactions and care plans efficiently and accurately.
  • Deliver medical services to patients, from assessments to treatments, both in-person and via telephone consultations.
  • Evaluate lab and test results, ensuring follow-ups for any abnormal findings.
  • Refer complex cases to physicians as necessary, ensuring comprehensive review and treatment plans.
  • Educate patients on health promotion, disease prevention, and ongoing health maintenance.
  • Conduct diagnostic tests and procedures aligned with advanced practice scope, including venipuncture and administering medications.
  • Interpret clinical data to form accurate diagnoses, developing and adjusting treatment plans accordingly.
  • Advocate for patient rights and participate actively in healthcare quality improvement committees.
  • Engage in community outreach and health education initiatives to promote health equity.
  • Maintain a safe working environment, following Legacy's safety protocols and procedures.
Qualifications
  • Education, Certifications, and Licenses: Current, unrestricted Texas advanced practice license; Active DEA; BLS certification
  • Proficiency with Electronic Medical Records systems, with a preference for EPIC
  • Experience: Minimum of 2 years as an Advanced Practice Provider
