About Position:

Job Summary: Under the supervision of the PACE Clinical Director, the Team Director has the authority and the responsibility to manage the performance of the interdisciplinary team (IDT). This position serves as the Center Manager on the PACE IDT. Its management responsibilities include overseeing the development, implementation, and monitoring of the plan of care; facilitating and coordinating care, including for transitions and service delivery; ensuring IDT compliance with the PACE regulations; ensuring high quality IDT performance and individual discipline performance; leading quality efforts in support of IDT performance and quality of services; and overseeing center operations in collaboration with directors and managers, with the goal of efficiency and high quality, services. This position is a subject matter expert of the PACE compliance rules, and works collaboratively with management to ensure all team members and the program overall are in compliance with these rules. This position will participate on the PACE Leadership Team and on quality and compliance committees and taskforces. It serves as the Adult Day Health Center Manager or substitute for this position.
Qualifications -
Education: College degree, at least Bachelor’s degree.in health related discipline preferred.
Experience: At least 1 year supervisory or administrative experience in health care and one year caring for the frail or elderly. Must meet the requirements set forth in the Adult Day Health Center (ADHC) regulations for ADHC Program Director.
Licensure, Registry or Certification Required: Valid NC driver’s license.
Special Training:
- Strong interpersonal, organizational, group facilitation and consensus-building skills. Must possess the skills necessary to provide care for geriatric patients and must have proven clinical leadership and management skills. Must possess the ability to learn, teach and research, as well as to solve problems and think analytically/critically. Strong oral and written communication skills are desired
- Must have the ability to get along in a team environment and to effectively handle difficult and unusual interpersonal situations.
- Individual must meet a standardized set of competencies for the specific position description established by Piedmont Health SeniorCare and approved by CMS before working independently, and must only act within the scope of his or her authority to practice.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
Pay Range: $81,761 - $109,880
