Nurse Practitioner – Job Description

Opioid Health Home Program

- Participates in the selection of strategies to implement evidence-based wellness and prevention initiatives.
- Participates in initial care plan development including specific goals for all enrollees.
- Communicates with medical providers, subspecialty providers including mental health and substance abuse service providers, long-term care and hospitals regarding records including admission/discharge.
- Provides education in health conditions, treatment recommendation, medication and strategies to implement care plan goals including both clinical and non-clinical needs.
- Monitors assessments and screenings to assure findings are integrated in the care plan.

- Facilitates the use of the EHR and other HIT to link services, facilitate communication among team members and provide feedback.
- Monitor and report performance measures and outcomes.
- Meets regularly with the care team to plan care and discuss cases and exchange appropriate information with team members in an informal manner as part of the daily routine of the clinic.

Staff Nurse Practitioner - Medication Assisted Treatment (MAT)

- Assess patients to ensure evidence of current physiological dependence, length of history of addiction, or exceptions to criteria for admissions are documented in the patient record, prior to the patient receiving initial medications.
- Ensures that a medical/mental health evaluation, including a medical history and physical examination, have been performed prior to the patient receiving the initial medications.
- Responsible for patient annual medical evaluations and 60-day medical evaluations on all patients prior to taking their medications; make referrals as necessary.
- Ensure that the appropriate laboratory tests have been ordered, performed and reviewed; sign-off on urinalysis results; review results with patients when necessary (ie., abnormal labs, continued illicit drug use).
- Review medication list and discuss drugs that should be tapered or discontinued due to interaction with medication.

Nurse Practitioner Job Requirements:

- Must be fully licensed in State of Michigan

- Must conform to acceptable practices within medical profession
- Must be familiar with treating the chemically dependent and mental health patient.
- Minimum of 1 year experience preferred.
- Hours of operation are (M-F) 5:00 am - 1:00 pm
- Participate in ongoing internal and external training to maintain proficiency in current trends relative to providing quality medical care to the patient population

- Ability to think critically and solve problems to create treatment plans best suited for each patient
- Confidence with "can do" attitude
- Effective interpersonal and written communication skills required
- A genuine team player with collaborative style
- High level attention to detail

- Ability to work independently and meet deadlines
- Compensation Range dependent on credentials/experience.
