About this job
*Assume primary responsibility for a nursing center's skin health support along with supporting the market's Skin Health Team Leads to hire, orient, retention them through education, coaching, and mentoring.
*Contribute to the national Skin Integrity Practice Council to report best practices, center needs, and/or recommendations to policy, education and guidelines.
*Participate in market initiatives with skin health and wound management.
*Lead and integrate a fully developed Skin Health and Wound management program.
*Use Evidenced Based Guidelines
*Own access to a robust formulary for all skin and wound interventions/treatment categories.
*Possess advanced technology for wound photography with automated incident/prevalence reporting available - no additional paperwork!
*Obtain immediate skin/wound training, support and coaching by national wound specialists.
*Ability to actively engage in enhancing the skin and wound program.
*Collaborate, integrate and support nursing center skin and wound efforts with an interdisciplinary team to include nursing, therapy, dietitians and others!
*Advance your skill set with advanced internal skin and wound trainings.
*Participate in a bonus program for achieving nursing center skin health and pressure injury prevention goals and objectives.
*Receive reimbursement for wound certification and annual fees.
*Work a desirable Monday-Friday dayshift schedule (as part of a nursing center team, occasional evening/weekend hours may be required)
*Must possess a RN license (MPT/DPT with leadership review/approval)
*Must complete Skin Health Team Lead orientation training and Wound Basics within 90 days of hire. *Commitment to attend monthly support calls is required.
*Must have national accredited Wound Certification (CWS, CWCN, WCC, CWON, WOC, AWCC)or be committed to complete Advanced Wound Training and national accredited Wound Certification within 1 year of hire. (For those hired with wound certification, completion of a fast-track hybrid program for regulatory wound care requirements will be provided.)
*Clinical experience in a skilled nursing setting is strongly preferred.
*Discharge planning experience is strongly preferred.
*Knowledge and experience with clinical charting, incident reporting and investigation response is required. *Must have a strong knowledge base of acceptable standards of care for nurses, providers, therapists, dietitians, and others on the nursing center team.
*Experience in team building and demonstrating respect for the interprofessional team in wound management.
*Must have knowledge of national wound guidelines and wound products.
*Knowledge of nursing home regulations and survey process is required.
*Strong Microsoft Word, Excel, Database Management, PowerPoint skills are required.
